New West Palm Beach Office & Expanded Clinical Coverage in Northern Miami-Dade

Life Care Concierge of South Florida is expanding its clinical presence across South Florida with the opening of our second office in West Palm Beach and expanded Nurse Care Manager coverage throughout northern Miami-Dade County, including Aventura and surrounding communities.

This expansion reflects the growing demand for sophisticated, highly personalized care management and our commitment to providing families with the clinical expertise, continuity, and responsiveness required to navigate complex and evolving care needs.

A Stronger Presence in Palm Beach County

Our West Palm Beach office strengthens our presence in Palm Beach County, where most of the families we serve reside. It also supports the continued expansion of our clinical team while allowing us to preserve the individualized attention and clinical rigor that define our practice.

As part of this expansion, two additional Nurse Care Managers have joined our Palm Beach County team, strengthening our clinical capacity and enabling us to provide greater continuity and responsiveness as our client base grows.

Their addition further  strengthens our ability to maintain dedicated clinical oversight while ensuring that families have access to a knowledgeable care team when needs arise.

Dedicated Clinical Coverage in Northern Miami-Dade

We are also expanding our clinical presence in Aventura and surrounding northern Miami-Dade communities.

A dedicated Nurse Care Manager currently serves families throughout the area, and an additional Nurse Care Manager is joining our team to expand capacity and provide dedicated emergency coverage.

Although our physical offices are in Boca Raton and West Palm Beach, our Nurse Care Managers provide a local clinical presence throughout northern Miami-Dade, working directly with clients, families, physicians, caregivers, and other professionals involved in their care.

For families, this means having a trusted clinical partner who understands the individual, knows the Care Plan, and remains engaged as circumstances evolve.

Our Nurse Care Managers  serve as the clinical anchor, integrating medical, functional, cognitive, psychosocial, and personal considerations into an  evidence-based Care Plan. We remain engaged across settings and  transitions, from home and hospital to rehabilitation, assisted living, and memory care.
